Student Registration Form

The registration form for new students is now available at our district website. Please click here to be taken to the Los Gatos Union School District registration web page where you can download the form in either PDF or Word format.

Medication Permit

All medications (prescriptions and over the counter (OTC) must be administered through the Health Office. A permission form, signed by the parent/guardian and physician indicating the name of the student, name of medication, dosage, time of administration and reason for the medication. The medication must be in its original container labeled with the student’s name and dosage otherwise it will not be given.

Note: This form must be renewed annually.

Field Trip Authorization

If you need a duplicate copy of the 'Parent Authorization for Field Trip' form you may obtain one by clicking on the "Field Trip Authorization" to the left. The completed form should be signed, dated and returned to your child's teacher.

Lunch Account Deposit Form

Children now have lunch accounts. You can add money to their lunch account by filling out this form.

School Driver Registration

If you plan to volunteer to drive students on field trips or on other school related activities you must first register your automobile. Please download the attached School Driver Registration form and submit it to the Blossom Hill School office.

LGUSD Student Internet Use Contract

Before a Los Gatos Union School District student is allowed to access the Internet from school, he/she must first receive age-appropriate Internet instruction at school, agree to the following rules and responsibilities, along with their parent sign the Student Internet Use Contract, and return the signature page to their teacher. Please keep this page at home to refer to throughout the year.
